In the applications to be made by third parties on behalf of the personal data owner to the COMPANY, the following Application Form must be filled and the power of attorney issued on the subject, and in the applications to be made on behalf of the children under custody/guardianship, together with this Application Form, this Application Form indicating the custody/guardianship relationship. A copy of the documents must be submitted to the COMPANY.
